  • Enter the email address you want to receive website inquiry, web form submission, and notifications from the website. If there are multiple email accounts, please list the rest in the "Note / Special Request" section on the next page.
  • Please provide us the email account you use for Facebook, Twitter, LinkedIn, Yelp, Google, and etc. If you don't have one, it is highly recommended that you create one using gmail or outlook. This email address doesn't have to have your "branded" domain. It is better to use a public free email service so that the account is still accessible without having to manage the "branded" account. If there are more than 1 email address, please list the rest in the "Note / Special Instruction" section on the next page.
